It appears to have been made with two separate pieces. The inside part, which prevents leakage, cracked around the plug and pushed in, breaking from the outer cap part, probably because it was tightened too much. The plug is still inside the cap and remains there. You may see some tiny specks of old glue surrounding the plug. The glue that holds the outer part to the inner is exposed, creating a little debris, but it should not enter the bottle. After cleaning the area, there should not be much debris left. The cap works fine.
